We appreciate that there are about 40 questions in the survey, however, if you have not received treatment then this should only take a few minutes to complete. If you have received multiple treatments, and most of the information is to hand, then the questionnaire should take less than 20 minutes to complete. Please note that you can now save your data as you are completing the questionnaire.
